How can I create a self-test like this one?

I'm hoping to get some suggestions on how to create the type of self-test found on this page:

Specifically, how to output this type of individualized results!
I know there are many quiz/test WP plugins, but haven't found anything that allows for the type of catered results found in the above example.
The overall results should be taylored or customized to the individual combination of answers selected. Not just a 'correct' or an 'incorrect'.
