Wordpress survey plugin question

I'm looking for a survey plugin for wordpress that will allow me to create a single generic survey and use it on multiple posts that will display results unique to that post.

Does anyone know of one that will do that?

    How it works is you create widgets with it and when you make a post you have the option to include them in your post.
