This is one of those questions that gets a "it depends" answer. Depends on your skill set and reputation but in general and I'm just throwing a rough estimate:

Freelancer from like upwork from india or a country in that area of the world do it for super cheap $5-100
    state side newbie 100-500
experienced web designer who needs work 500-1500
highly reputable freelancer or design agency 3000+

That is just what I've seen in my experience, those numbers a completely generalized though. In my career as a freelance web designer/developer the lowest I charged was 250 and the most I charged was 1500.

Depends on your skill set and reputation. It's also depends on your requirement, Large or small Project. but in general and I'm just throwing a rough estimate:
Freelancer from upwork around asia. Fiverr is much cheap.
    super cheap $5-100
    Static side newbie 100-300
an experienced web designer who needs work 300-1000
highly reputable freelancer or design agency 1500++

Fiverr and PPH Some designs have good feedback are needed, they can work at a lower price.

Just design or programming? Designers are a dime a dozen these days. Are you a skilled graphic artist that does custom artwork? Can you create a website from scratch or do you just assemble pictures on a WordPress site? Are you able to program custom JavaScript features or do you just cut and paste other people's plugins?

Like others have said, you can get offshore people for $15 per hour to build you a throw-away site that looks like everyone else's. On the other hand, I've seen a large company pay two million to a big agency for a site built on an enterprise grade content management system (and I wasn't too impressed by the site). It all depends on how you market yourself and what you bring to the table.

$500 and under is outsourced from india, or cheap local student or somebody looking to start out. $3000-5000 can either be a ripoff agency or somebody over-charging...or considered cheap for a REAL design firm. The absolute best guys out there won't touch your project for less than $6-10k.

It also depends whether you're referring to a template/theme that you can edit your self (fairly easily even for a newbie I might add) or a completely white glove done for you and one of a kind/not created from an existing template site....
