How to build a SIMPLE payment form that includes several payment options?

Hey guys, I'm having some difficulty in this one seemingly simple task.

I have had my website up and running for over 2 years now. It's a decent looking site, and I managed to build the entire site by myself. I'm not much of a coder though. I just did what I needed to do in order to get the site up and running.

I have a "course" that I sell on my website, which is a one-time payment thing. All I did was simply add a PayPal button, and it immediately takes them to PayPal's checkout page. There was only one payment option - lifetime access for $47.

I want to change to a monthly subscription plan now though, which gives 3 different payment options. I could simply just have 3 PayPal buttons, one on top of each other with a price next to each one. However, that would look kind of ugly.

I'd like to have a nice-looking "form" where I can have my 3 different PayPal buttons incorporated such that the user can select how they want to pay. I don't know why I'm pulling my hair out trying to figure out how to do this though.

Can anyone help?
