Wordpress: Is there a way to make a request form page?

Hello,

So I'm putting together a review site and I'm in need of a way for folks to submit requests for potential reviews. Anyone know of a plugin or other way to do this?

    How about this customizable form

    Mainly used for Contact Pages but adaptable for most request submissions.

      Looks like that might be suitable for what I will need. I'll have to check it out once I get everything up and running. Still debating between which review plugin I want to use. Thank you for the share.

    As pjCheviot said, I'd give contact form 7 a try. It is simple and effective, and you can add captcha functionality to it easily. I use it on my own sites and have never run into troubles with it.
