I need a banner designed

Hi, I need a banner (2 sizes, 1100x900 and 750x550)

Basically it should say:

Do You Shop at WalMart?

Would You Like To Get Free Groceries For Life?

And have a picture of a WalMart Gift Card, and a cart full of groceries, maybe a happy woman.

Also a call to action button that says: "Click Here"

animation in the banner would be a plus.
