Can You Help Me Sort Thru The HTML & CSS mess ???

I have a page on my site that I was told by aweber that is distorting my web form. I had to resize it using drag & drop..& must have corrupted the code. I need help badly and now.

http://www.nylez-affair.com/enter I like the form I created but how can I get a clean code and use it and also direct it towards aweber ?
Aweber gave me this but i don't know what to do

    You just need to use the code that they provided in the link and then customize to point to your current form's settings. This is pretty easy to do just look for values between " " and then insert the appropriate value from your current form.

    If you have issues let me know and I will handle this for only $25.00 which is half off of my normal rate of $50.00 per hour.

    Several options: load your HTML and CSS code into an editor so you can properly format the structure and tags; use an HTML validation service from W3C to identify potential problems; properly comment the HTML and CSS code so you can easily replace blocks of text (like the "insert this code" you might receive from aweber) without much hassle. Curious to know what solution works for you. Let us know. Thanks.
