Hi Fellow Warriors, I designed my site with frontpage. I have published it but I'm having problem with the contact form on the contact us page. The field names are not showing, what is showing at the top of the form is [FrontPage Save Results Component]. If I click submit button, I receive the following message "Not Found

Hi Fellow Warriors,
I designed my site with frontpage. I have published it but I'm having problem with the contact form on the contact us page. The field names are not showing, what is showing at the top of the form is [FrontPage Save Results Component]. If I click submit button, I receive the following message "Not Found
The requested URL /_derived/nortbots.htm was not found on this server.
Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request."
FrontPage Extentions are installed on the server. I can't really figure out what is wrong. Any assistance in solving this problem will be highly appreciated.
