Looking For WP Plugin For Opt-in -> download

I have a downloadable free ebook, so i'd like to offer it to my visitors. I'm looking for either a popup lightbox solution ("fill in your name and email to download this book") or a sidebar widget. i prefer the lightbox variant, so i can use it when they click a very big and attractive button.

Maybe some advaced link building plugin or what not.

Any tips or suggestions?

Didja sign up with Aweber? They have sample javascript code you can use.
