What is the Best Wordpress Contact Form plug in?

We are building a new wordpress site and need to incorporate a Deal Submittal Form. An example is as follows:

Submit Capital Request

Does anyone know of a good plug in to allow me to incorporate something like this into our site? Thanks!

    I would go with Gravity forms, it is worth the money. I use it for all my businesses - including a very similar form for resume submissions, deal submissions, etc. If you need more info, just PM me.

    Though, a free alternative is ContactForm7. However, it requires more effort to setup. Seeing your form, Gravity is the right way to go.

      Maybe this is also worth a look: MyContactForm have a specific tutorial for putting their own forms on Wordpress, (also ebay. your own site whatever) if thats any help.

      You can host the whole shooting match with them if you like. Autoreply, archives all your contact messages etc, and redirects contactee to any page you choose. Full featured, you could say.

      I use them for all my web sites. But Note: There is a low contact maximum number limit per month on free accounts (it might be as few as 25) and up 5000 contacts each month on paid accounts, which are about £20/$25 a year.

      Highly recommend them, put my money where my mouth is and in our second year of paid upgrade.There is no limit to the number of forms you can create, plenty of templates and examples etc.
