What Web Form Does Warrior Forum Use Top Right Corner?

One of my clients wants a web form like the one Warrior Forum uses on the top right hand corner of this forum

Does anybody know what was used to make it?

Someone told me its a aweber form but doesn't look like it to me. I know its connected to aweber but the form it self looks like a customised form using some sort of software to create it

Nothing terribly special about it. Just a few gif images and some padding to line everything up in the box.

Using Chrome, I can just right click any of those images, click "inspect element" and see all thats going on behind the scenes quickly.

You should be able to recreate something like this with Aweber or any other email program pretty easily.

  Not really sure what else anyone can say about that. It's an aweber form surrounded by some small custom graphics - those could've been created in Photoshop, Illustrator or any other image editing program and it's all included as part of the table row that contains the header images.
  
  I haven't used Aweber in a bit, but I believe they make it quite easy to put together your own graphic signup form that you could then tweak to do whatever your client wants.

that's a newsletter plugins if you used wordpress.

Sorry I thought you gonna used it in wordpress.
Its aweber program, warrior only customized the form using html css and graphic designing tool such as photoshop.
