How to build an affiliate capture page using wordpress?

hello fellow warriors..

I am trying to figure out a way to build a capture page using wordpress that also allows me to track which affiliate generated the lead.

Here is what I am trying to accomplish. It doesn't have to be wordpress, I am just familiar with that platform.

I sale telecommunication services offline and have phone vendors and equipment companies that send me business. I will then pay these companies a monthly residual commission for the referral based on the customers telecommunication bill. I am trying to make it more streamlined so that each vendor will have their own affiliate link to promote my services, mainly just a capture page to get the customers info so I can contact them and provide them with my services. But I want to be able to know which vendor sent the lead so I can give them credit for the sale and pay their commission. I am not looking for anything that pays the commissions as I already have that built, all I need to know is what vendor sent the lead.

Does anyone know how I can do this?

    I remember seeing this done in analytics by setting up goals. Basically one goal would be for the visitor to land on the capture page and goal 2 would be the visitor landing on the conversion page. It would then track it. Now I'm not sure how would be able to make unique links for each affiliate.

      That's not quite what I'm looking for. It needs to be easy for the new affiliates to sign up as affiliates. I'm thinking more like a plugin that I can use that will do what I want. I have found a WP plugin that will turn my site into an affiliate program and also a WP plugin that helps build capture pages. I'm just not sure if they are compatible together. And if it will work the way I would like it to.
