WP HeatMap (free) theme - Opt-In form problem

Hi guys,
I am using the free HeatMap WP theme on one of my sites but when I enter the html for my web form in the text widget at the top of the sidebar, it doesn't seem to work... anyone got any ideas where I may be going wrong?

If your form code contains PHP code, you might have to install something like WordPress › PHP Code Widget « WordPress Plugins or WordPress › Exec-PHP « WordPress Plugins first, to allow that.

Are you sure you are doing it right? Which mailing list service are you using? Aweber has a very easy method for adding forms. Mailchimp is also easy.

Who can help me find a person who does capture pages and a review page and 7 followups! Sorry has nothing to do with this but im in the right forum, I am also having hard time! Can anyone here get me a good deal? Also in couple of days need some backlinking done. One or the right now will help or both.
