Can you make my Opt in button work?

I'm hoping someone can figure out why my opt in button on my new squeeze page won't work.

I'm using Profits Theme on Wordpress and they have some squeeze page templates but I don't really like them. So, I designed my own squeeze page and set it up on a regular "page" and in order to get an opt in box on it I copy and pasted the opt in box code from one of their squeeze page templates.

Well, it looks great but when I press the "submit" button after entering a test name and email address, it redirects to I-contact's error page. I'm not sure what's wrong with the code, perhaps something didn't copy over correctly.

I would prefer to give someone who'd like to help the website URL via inbox.

    Feel free to send me the URL.
    It's likely the form you've copied is posting the content of the form to some other file that's supposed to process it. Which apparently you haven't copied. But that's just a guess.
