Custom Forms WordPress Plugin?

I need a WordPress plug in that will allow me to make a custom form for my writing site. I've found a few that are just for 'contact us' pages but I can't seem to find one that allows me to make the form just how I want it with several unique fields and maybe a drop down box.

I want to use it to allow customers to submit a request for service. It should be able to include things like a box for the writing's purpose (SEO article, Sales Copy, Twitter blast, ect), a box for the number of words, something for the keywords (if any) they want to target. Then a larger box for general instructions.

I can't imagine this should be that hard to find, but I've installed and removed 4 or 5 plugins already trying to figure it out with no luck.

    • Thanks! Those look great. I was hoping for something free but I guess $39 is not so bad.

    • Ive used contact 7 wp plugin myself and its pretty simple and can do what you want.

      you customize the form in your wp contact 7 dashboard,

      and it will generate a short code for you

      so in the pages where you want the form to appear just insert the shortcode

      example

  • I use Contact Form 7 as it's very customizable, free and includes:

    Text field
    Email field
    Text area
    drop-down menu
    Checkboxes
    Radio buttons


    There's some more options there too, I recommend it.

    Try using "Form Manager" which you can get here: Form Manager


    I am using it on this site of mine Di-NamicDesign and if you play around with the various radio buttons, depending on your answers, additional fields will pop up. It's totally customisable.

  • C-forms is also nice. It save entries into a database. If you want to know how it looks go and google cforms for wordpress. Try it, its very nice....I use it in all my websites!

    • I have to agree with JDunhin. C forms is superb and will do exactly what you're looking for.
      I strongly suggest you try it as soon as you can. You'll love it.

    I use contact form 7. So far I have been able to do almost anything with that plugin.
    You can download it from here wordpress.org/extend/plugins/contact-form-7 or you can just search it in your plugin tab inside your dashboard.

  • Don't forget the simple, free, and very easy to use Google Docs forms... has a wide variety of pre-designed styles... all data is captured in a spreadsheet.
  • I've used Visual Form Builder. It's FREE, but there is a pro version for just $10.

  • contact forum 7 is good.

    Also, if you have some non-editable contact forum, you can just go and edit the raw HTML yourself to add in new <forum> fields. Basically just a copy and post and change the ID="".

  • If you want to install form apps, here ones of the best site that offer multiple contact form. But this apps is paid version. You can try widgetbox and select the best one contact form apps for your blog.

    • Not a wordpress plugin, but I've had Wufoo.com recommended to me many times. I've used it for some short forms, but they won't let you do much for free, as I found out just tonight. So, I did some looking around, and it looks like Jotform.com allows more than Wufoo does for free, and their paid service appears to be cheaper too. This might not help, but is just a suggestion because I stumbled upon this thread while searching for form information.
  • use contact form -7 as and style it using CSS or gravity forms for advace solutions, only two plugins that I recommend for my clients
