Help needed with registration form, search form and login form, so not much!

I had a splash page made for me for a white label dating site which has a registration form, a login form (username and password) and search form. The manager of the white label company sent me the code for making the forms work only the splash page designer wasn't sure how to do it so I told him don't worry, if he just does the design I will take care of the rest.

He has done a good job and I am pleased with how it looks but now I've come to fill in the missing details and I'm stumped. I've hardly done any html before and know next to nothing about coding or adding scripts or whatever it should be called. I had a play about but it will take me a fair while without some help, or somebody to do it for me, for a price of course.

I would like to figure it out myself as most times I eventually do but unfortunately too much concentrating and staring at the screen now makes my eyes and my brain ache so before I try somewhere like Freelancer or something I thought I'd ask here if anybody has a clue or could possibly recommend somebody who can be hired for the job.
