Did you recently install a new theme or plugin?

Start by deactivating your plugins and see if it goes away.

If it does, start activating them one by one until it comes back. If it was a plugin then replace the plugin with a clean version.

If it's there with all plugins deactivated, and you switch back to the default theme in WordPress, then your install is likely corrupted or hacked.

I would not delay doing this....
If you've been hacked you need to stop any further damage quickly.

Ok, I deactivated all of the plug ins one by one and it appears that the Better WP Security was the one that was causing this. Is it possible that this plug in is corrupt or is it that that is the way this plug in functions? I am asking because it was quite a bit of work to set up this plug in so I was just wondering if anyone knows before I go through all of that again.

    Thanks!

Correction: it was the Bulletproof Security plug in causing this. Sorry

Well, it's great that you found it so quickly.
I would make sure your running the latest version of the plugin and WordPress.
If you are, you could replace the bulletproof security with a clean copy but NOT through wp-admin area.
If you delete it in the admin area you will lose your settings.
      • Login to admin and just deactivate the plugin (don't delete)
      • Download a clean copy and unzip it.
      • Login in to your server via FTP...
      • Navigate to the plugins folder...
      • Delete the bulletproof security plugin folder...
      • upload the new clean copy of the plugin.
      • Go back to admin and activate again.

If that doesn't correct the issue, you might send a support ticket/email to bulletproof security.
It's a free plugin but they still offer some basic support in the wordpress forum.
There are so many settings (as you said) for this plugin, I just can't remember if anything should display in the toolbar, but based on what's displaying, it doesn't look right.

Hmm no experience with that plugin... how many plugins did you have installed?

I have 18 plugins installed - five of which were security plugins. I think I might just delete the problem one and see if the rest of them will be enough. I always keep my plugins and my Wordpress versions up to date.
