Web design/development reporting

Hi, I am looking for a professional web design/development sample report. The report has to include information on the purpose of the website, how and why it was build this way, how it works etc. I looked throughout three or four pages on Google but I did not find what I was looking for, so I hope someone from here can help. Thanks in advance!

    • Not quite. I am looking for a project documentation template with professional formatting, which I can use to describe what I am doing during the web design/development stages, why I do it and what does it do, so I can give it to the client when his project is finished. Thanks anyway!
