It really depends on whats yoru interest and what you are good at, If you have good creativity then you can go for design and you are good at logics then you can go for programming.

Or should i design first and let the programmer know I want the site to look like this on the front end? Ask your programmer. As a programmer, I tell my clients that they need to provide a design or designer because I have zero design talent what so ever. But I have colleagues who are great designers and programmers in one. Depends on the person. See if your programmer can design and if not, hire a designer.
