Graphics are your friends: perhaps some before/after pics, or some catchy teaser about the treatment?

Wanna becareful with before and after pictures, real people that used the program, or you could get in trouble.

I agree, the submit button looks bad, well you know that.

Biggest thing is the bullets are boring. They read more like features rather than benefits. Need to punch them up with more exciting, benefit ladden words. Get them wanting it bad.

You probably also know you should get more opt-in's if you only ask for the email address. just a thought.

Try using png file format for small images and jpg file format for big ones so that your loading time would not bump on big graphic size. Just a suggestion though.
