I know there are different kinds - some that look cool, others that look plain. Can one create one for a website or do you guys recommend buying them?

    Before spending money, check out a few things you can get for free. My recommendation is called cforms from Delicious Days.

    I've used them a lot to replace the comment box provided by WordPress, but you can certainly use cforms in other ways. Let us know what happens. Thanks.
