Looking for a Contact Form With Unique Features

I'm looking for a contact form (a Wordpress plugin would be perfect, but I can implement regular scripts if necessary) that would do a couple of unique things:
1. If a person enters an email address with a particular domain (widget.com, for example), then the form would record the person's details in a database (and/or send me an email), send them to a page that tells them to expect a reply, and then send an autoresponse to that email address.
2. If a person enters an email address with any other domain, then the form would not record the person's details in the database (or send me an email), send them to a page that tells them that the form will only accept email addresses with widget.com as their domain (or display an error message directly on the form), and not send an autoresponse.
Anyone ever hear of such a thing?
