Google index with wrong IP Address...What to do?

This is really weird. Under one keyword I one of my sites is listed as normal, but when I click on the link it goes to a URL like this

http://216.246.78.220/
and it goes right back to Google. That was perhaps the old IP address of the site, I don't remember. The site was moved about 6 months ago and the listing in Google uses the real url, (http://www.sitename.c ) not an ip address.

Why would this happen? Further, what can be done about it?

    This is really bugging me.

     <a href="http://www.mysite.com/" class="l" onmousedown="return clk(this.href,'','','res','4','')">
    Almost all my indexed pages do the same thing. When I click on the SERP in Google I get bounced back to Google. I noticed that a link that should be like

    http://www.mysite.com/stuff/morestuff/1/here
    ends up like
    HTML Code:
    http://www.mysite.com
    What could be going on? No one that searches can get to my site.

      Got it. Geez.. For anyone else that runs into something like this.

      it was an .htaccess file that was changed by a plugin. It really messed up my site.
