Need to add an advertisers webform to my site.

I'm designing an affiliate site where the prospect will have to fill out an application for services. I would like to copy the advertiser's webform from their page and add it to my site. That way the prospect doesn't have to go off-site.
My intentions are to have my affiliate cookie fire when they hit my site and when they fill out the application the submit button will send the info to the advertiser's targeted link as if they clicked on my affiliate link and filled the form out on the advertiser's page.
I basically don't want the prospect to realize I am just the middle man.
I am also interested in being able to keep a copy of info the prospect is sending to the advertiser.
Can anyone point me in the right direction to accomplish this? It would be much appreciated.
