.htaccess Experts How do you make this log in box?

Hi,

I went to a site today that was password protected. How do you add the extra password window? I thought there could only be two, they have 3 could you have 4, 5 or more?

    It normally just username and password.
    Google: "htaccess login box" and read the found pages.
    You need a .htaccess file in the folder and a .htpasswd file outside the folder.

    I don't belive that is a htaccess/htpassword generated box.
    It looks like a windows generated box.

    htaccess only works on *nix servers so if it is a windows server then you have to setup password protection differently and I think that is where that box comes from.
