It doesn't have to be a table. Wrapping the first element after <body> tag with link should work.

Your global postback looks correct.

Did you test the campaign URL from the link in campaign setup page? IIRC CPV Lab doesn't record visit/click from the admin section to avoid skewing the stats....

I second Primo PDF. I had no problem with affiliate links inside the Word doc, they are converted to links in the output PDF file. Is there any settings in DoPDF that maybe disable hyperlink?
