Well, pretty easy you just need a the MySQL backup files from the current host and upload them to your new DB.

Ask your host if they can help and if you don't have Cpanel or PHPMyAdmin Access.

You should use a 301 or 302 redirect if you do that, this will avoid google to penalize you. make a seacrh on this in the SEO forums, they explain very well how you can do that correctly.
