LeadImpact Landing Page Disclaimer Text

Hey there...WarriorForum newbie here.
Created my first LeadImpact landing page (instead of direct linking) for a PPV offer and the campaign was denied because I did not have a proper disclaimer. Any recommendations on decent verbiage for the disclaimer? Tried looking in the archives but could not find much.

    Just Google a basic disclaimer. It really doesn't matter what it is as long as it's there.

    If you are promoting free ipad offers just include "We are not affiliated with, endorsed or sponsored by Apple or Apple products." I also Include "participation required" below the free word.
    It all has to be above the fold or else they will reject.
