How to change html to php to insert template in Wordpress?

Hello!

I've just had a squeeze page created on fiverr. The page was done in html and I have received:
- 1 image file
- 1 index
- 1ccs sheet

I now need to install it on my wordpress blog. Can someone tell me how to do this (if it is not too complicated) or is best to let Fiverr do the additional work?
