Ebook formatting software? I want to turn notes into a e-textbook

I am creating a review course for a certification and I want the textbook to look very professional, with the ability (for me) to add notes in the right margin. I can use ms word for this, but I want something that will give it a more crisp, polished look (like presi.com).

Can anyone recommend something like this or should I go through a publisher? A lot of my sales will be driven through discussion forums and I will need people to really enjoy the product and recommend it.
