Question about Mastermind Guidelines

Does anyone have particularly good guidelines for running a mastermind group?

Especially for running one online ? Though I'll also take guidelines for face-to-face groups. I'm pretty sure the topic is covered some place in Think and Grow Rich, but I'm not sure where and I have the audiobook and can't quickly figure out what CD it's on.

    Can anyone who participates in a mastermind group tell me:

    - how often do you meet?
    - is it in person or on the phone or online?
    - do you start each meeting with a particular question to either the group or an individual
