    Some people are medically obese and are healthy. Some are skinny and can be worse.

    Sure.

    But you are always healthier if you are not grossly overweight. It's just such a strain on your heart to carry that much extra weight all the time.

    If you're 6 feet tall and weight 180 pounds, you'll live longer than if you weigh 280 pounds.

    If you define healthy as being free from illness, then yes, you can be both overweight and healthy - as long as you're able to be as active as you want or need to be.

    But excess weight carries an increased risk of developing illnesses or diseases, and also of physical injuries.

    You also have to consider the mental health of someone who's obese and worries about it.

    I think the optimum word in the question is "remain". And the answer is no. While an obese person may currently show healthy markers from test results, the excess weight will catch up to them in the form of joint pain, back pain, increased risk for certain cancers, diabetes, heart issues, skin problems, COVID, mental health issues, etc. It's generally just a matter of time and is affected by genetic, social, and environmental factors.

    Go to a retirement home and see how many obese 90 year olds you see. The ones you find are genetic anomalies.

    well the same behavior fueling the obesity is causing other health problems. Or other health conditions are fueling the obesity weather it be medications for the physical and mental illness. Or physical injury that causes reduced physical activity.

    Now many body builders will low body fat are considered medically obese. So it matters on the individual level versus a BMI based obesity.

    How much muscle and how much fat. Visceral fat the stuff around your organs is generally just a bad thing the more obese one gets the more chance for there to be a lot of that type of fat.

    Nature often cures obesity by making you lose weight by giving you diabetes because to be obese means you overeat the bad stuff. Thanks to diabetes I have never been slimmer.

    Frank mentioned the mental health side of it. I think that when/if sex gets harder, can't go run with the kids and play with them, suffer from low-self confidence, lose job opportunities, etc. it may be time to look at possible changes no matter what the blood tests, echocardiograms, or stress tests say.
