Embed video in mobile website

I've been trying to embed a video in a mobile site that will play on my Android and I just cannot seem to get it to work. I know its possible because I have scanned QR codes that go to landing pages that use video. Does anyone have any experience doing this sort of thing?

I use h.264 codec with the newer html5 format, and it seems to play the videos on most phones. Basically using software to create an mp4 file that is formatted for both web and phones. I use Adobe AfterEffects CS5 and Camtasia7 to save the video file in the h.264 mp4 format. Then I upload the video to my EasyVideoPlayer.com account. Then I just paste the formatted html code into the web site.
