There are spambots that roam the net looking for forms to fill in - blog comments, contact and support forms, link and content submissions, it doesn't matter. They scan the source code for the <form>...

yes the lead capture page is the first thing I setup when going live with a website. If the sole focus is to generate sales collecting leads is vital.

When you are using the domain name without site operator you are actually going to get some backlinks to the page with indexed pages of the site. I would recommend you search using intext or intitle...

OP, if you can get the urls of all your website's pages, you can paste them into scrapebox and check for indexed in either google or bing, using the controls on the right.
