Create Form That Allows Adding Photos

Hello,

I have a roofing company that's a customer of mine and he wants me to create an online form where he can add photos and notes.

Basically, he wants to go up on a building's roof and take some photos of necessary repairs. He then wants to go back to his office and write up a report showing the photos along with some small notes underneath the photos. He'll then print it out and give it to the building owner as a repair proposal.

He's not very computer literate so having him doing it in Microsoft Word isn't really an option. I was hoping to come up with something more elegant such as him just filling out an online form where he can just drop in the photos and notes.

I tried using form building in Adobe Acrobat but I can't get that will work.

Does anyone know of a program which allows me to make an online form (or even just on his computer) that's a template where he can just drop in photos and notes?

    Have you had a look at Jotform.com? It's a quick form builder with a lot of options. I'm not sure if it has a report option like you want, but it's probably worth taking a look.

    If Word is too hard, honestly, he's probably screwed.
    Online forms are always harder.

    Probably the best approach would be to have your client send the pictures directly from his phone. If it weren't for the privacy concern, you could simply do this with Twitter. I've got to imagine that there's a similar service that keeps the pictures private. If not, there's a huge opportunity for you!

    {Googles a bit...} Looks like this might meet the need. Use that in combination with a "private" email address (like "your.actual.email+roof.repair@gmail.com") so you can easily trigger a Gmail label, which then apparently triggers this extension that saves the file to your Drive area (that you'd share with him). [Disclaimer: I haven't tried this; I'm just telling you what I saw in that article.]

      Oh, here's a similar idea using Dropbox. That article also mentions Jotform (as did saxatwork above).

        Thanks for the replies everyone. I did look at some of the suggestions and through my search I found a program called LiveCycle from Adobe. You can create forms with it (including having the user add their own photos).

        It will then save it as a .pdf file.

        Unfortunately, it looks like you may need Acrobat Pro (I downloaded the free trial of both programs). We'll see how it works with my testing and if my customer.

    Well, you could always design a form in wordpress using html5 not too hard to do, and have it either print out for printer or email it. *shrugs* Good luck

    I need to make a Google Doc form, in which the users need to upload an image. Is it possible to do so using Google Docs form? If yes, then how?

    hi christina21maria, just google form+. it allows you to upload images. I think with free version you can only create 15 forms monthly.
