Need help with a form script on website

I have a chiropractor client who would like people to be able to make appointments on her website. I see the forms out there - have not created one myself - would like to know what you experts recommend - Any suggestions?

Is this something I could purchase from a warrior? do myself simply or purchase from an outside opt in vendor? They seem simple enough - however I am not sure about this.

    This is definitely something that anyone could do. Look into PHP forms. You could master them in a matter of a day.

    However, with appointments you often need a system in place where current appointments are held in a database and users can only book free slots. So not sure how advanced this form needs to be. If you need an advanced one and you have limited programming and database knowledge I would highly suggest hiring someone for the job.

      Thank you - it is definitely this type of form she wants - space available / free slots. My programming is limited to I will seek out another source. I can do simple forms but don't want to spend that much time on this learning it all!

      I am happy doing simple forms but don't want to take the time at the moment to learn a more complex data building one that displays available appointments etc. Looking for an easy way out!

    Hi Terry

    Would a simple e-mail form suffice? If she is using WordPress for her site there is a free plugin (that also has a commercial version) called MM Forms that I've used a few times with success:

    MM Forms | plugins @ motionmill dot com

    Otherwise if it's a static site it is a fairly basic design task to make a simple e-mail form. You could build one (or have one built) yourself or make use of an online service that does it for you:

    Email Me Form™ - HTML Forms, Web Forms, PHP Forms

    Website Contact Form Generator - ASP, PHP, CGI Email Form Scripts
