Issues with a contact form in wordpress

I'm using a wordpress plugin called DDFormMailer to make a contact form/booking form on my site. It's a pretty involved site (for booking holidays) and it collects a lot of info and it's been workign really well.

The form is currently returning to 4 email addresses.

But... sometimes it doesn't. Sometimes 3 addresses will recieve the form results, sometimes only 1, sometimes none. And sometimes it works just fine.

I've contacted the author and had no response.

I just don't even know where to start looking. Is it likely to be the fault of the script? Could it be a server issue? (Have already checked spam filters on the email accounts, so not that.)

Any ideas on where I should start...?

    It's likely NOT to be the fault of the script. Generally, a script either works or it doesn't.

    Email problems are maybe the most difficult things to diagnose because there is so much that can go wrong, and most of what can go wrong is beyond your control.

    As a start, you can see if your hosting provider will provide you with email server logs. Better yet, see if they will look for you -- reading email server logs is like trying to read Swahili (assuming, of course, that you can't read Swahili ).

    Hey Jo,
    Its quite hard to tell the actual bug in your code before viewing the code I agree with Johnsan if script don't work then It will not work at all,
    why don't you try formbuilder its free and you can add multiple receiver email address,
    I have added to my website which is in my signature check it out I hope you will like it.

      Another great option is cFormsII (free plugin at WP).
      I've built many custom forms for clients using this plugin.

        Thanks all for your thoughts.
        My hosting provider informed me there is a php fatal error with an undefined add_option function - with one of those functions being the recipient field of the email.

        This scares the pants off me, but does seem to suggest (to my mind which is completely empty of any php knowledge at all) that the fault may indeed be in the plugin code.

        Think I'll give some of these other plugins you've all suggested a try.

    I think you using the basic mailing system that why they send you few mail in your email box
