PopUp Domination Not Hooking

I just installed PopUp Domination for the first time and I am trying to use it with MailChimp.

I copied the HTML code from the "Signup Form Embed Code" in MailChimp to the plugin. However, the plugin doesn't seem to capture the email field, as I circled in this picture:

I copied only the code between the <form> tag (including the tags).

What am I doing wrong?

      Oops, I should have been clear. I am copying everything, including the tags, into the plugin.

        I got it working to the point where the NAME field hooks, but EMAIL field still doesn't hook. I have a feeling the form code was recently updated and the plugin can no longer read it correctly.

        Can someone do me a huge favor and send me a copy of their working signup form code (without your account ID of course). I can try that out. Thanks in advance!
