Help with form creation

I am in the process for setting up a website for lead generation.

I am needing help setting up a form that once a visitor enters their email
their info will be sent to the following:

admin@leadgensite.com

as well as:

lead buyer 1
lead buyer 2
lead buyer 3

Hopefully someone can help me get this setup.

What would the rates for some work like this be?
