WordPress "Tell a Friend(s)" plugin wanted

PROGRAMMING

Hello.

I'm using a WordPress 3.3.1 and I'm searching for a simple plugin, which would allow me to put a "Recommend to Friend(s)" box to any page/post I'd like.

I need:
-option to set up to five friends' email boxes
-chance of writing my own email message recipients will get.

basically a box with

My Name:
My Email:
Friends Email:
Friends Email #2:
Friends Email #3:
Friends Email #4:
Friends Email #5:

[SEND]

Do you guys know of any free plugin I could use? I tried several of them, but still didn't come across the right thing.

Let me know if you find one... If not I might code it.
Explain more about the functionality.
How will it look on the front end?
Is it to go in the sidebar? or on all post?
Shortcode?

  Yes, it should be an "all-post" type, not for the sidebar. Eventually it would need a shortcode and where you'd put it, the boxes would show up, just like I said -

  Friends Email #5:

  and the message these friends would receive should be editable inside the admin panel of the wordpress.

  Looks like Gaetanoc is right about super-refer-a-friend.
  
  It is the only plugin that appears to do what you want.
  
  Unfortunately, it hasn't been updated for just over a year, only has a rating of 2.83/5.

so a simple email form with up to 5 email address.
The user adds the message.
I would think it need the link to the page automatically added.
Something like:
NAME wanted you to see <a href="page it was from">this<a>

NAME says:
the message here....

BUT.....

How to you want this to happen. I am thinking a nice little icon that appears with a shortcode? Yes no maybe?

I will try to get this done tonight.

After thinking about it, I am going to append it to the content. nothign for the use to do that way.

Plugin is done... who wants a copy?
Send me an IM and I will send it to you. Need a few guys to test it.
The css is minimal, easy to edit in /tell-a-friend/css/style.css

All ajax, validates email, if good send the email. Page never reloads.
"tell a freind" button is added below the post.
Clicking it opens the email form.

As far as i know there must be a plugins to something like this in wordpress. So you should just find it in the wordpress site and choose the one you need to use.

  makes it hard to refer friends.
