How to add this:
<form method="post" action="cant post links" target="_blank">
<table border="0" align="center" cellspacing="0" cellpadding="5">
<tr><td colspan="2"><font size="2" face="verdana,geneva">Fill out your e-mail address<br />to receive our newsletter!</font></td></tr>
<tr><td valign="top"><font size="2" face="verdana,geneva">E-mail address:</font></td><td valign="top"><input type="text" name="YMP0" size="20" /></td></tr>
<tr><td colspan="2"><input type="submit" value="Submit" />&nbsp;</td></tr>
</table>
</form>

to this box:
upload.x-ee/image/2468008/themehelp.PNG/cant post links (remove x and - between link)

    I can perfectly see your image. Just edit that image in Photoshop, slice out the images and then code the html to look like that and integrate the form.

    If you need help with all of that, shoot me a PM.
