Need Form Builder With Conditional Logic + Shortcode Capabilities...
I need to create an interactive multi-choice form.
The form needs conditional logic abiliites that allow for only 1 question to be shown at a time.
I also need the ability to write the form entries to a cookie. (If the form allows for shortcodes then I should be able to do that with shortcodes + javascript on the thank you page.)
So much the better in the form also has backend analytics.
So far I've found a builders with some elements, but not all.
Anyone know of a form builder that will allow for this function?
