Hi!
I use what the page is about: treatment, book, contact etc
and then "massage city" + company name
Is this template ok?

So it looks like this:
Treatment | Massage city | Company name
Book time | Massage city | Company name
Contact me | Massage city | Company name
#correctly #title

I dunno, its not clear to me so I would recommend revising. My advice is to look at what your competitors are doing and use their title and meta description as a template to build yours.

I don't think it's necessary for "massage city" to appear in every title. So, I would remove it from the titles (that's the only thing I'd change).

Edit: Also, single words in titles aren't descriptive enough unless they are on category pages.

How many massage parlors do you own?

Book time and contact pages are non issues when talking about
    titles.

Most people put company name first.

It is certainly not optimized for the search engines. Instead of using these separators, use the exact keywords that people actually search for. So it should look like "abc treatment in the city xyz".
As a side note, this exact keyword should appear in the URL, heading tags, body text and link text too.

I wouldn't use Massage city in the title. Also would post the Company name first... For example a title:

    Contact Me | Company name

is not so professional... I don't know why, but...
