Long tailed keywords got me confused!

Long-tailed keywords basically do what they say on the tin - they're a combination of three of four words added to your copy so that when someone types in more specific search request you will rank higher. So instead of 'mechanic' which there will be lots of completion for, you could add 'Ford mechanic in Castle Hill' which means that you'd be much more likely to appear at the top if anyone where to type that into Google.
But what I'm wondering is who sets these? I've never seen any in any of my keyword search research so is it just a case of deciding what fits and running that through your copy?
I can completely see the benefits in using them, I just don't know where people get them from!
