From what I understand, the remove URL tool only temporarily removes a URL for 90 days. The only way to permanently remove a URL is to either remove it from your site, password protect it or use the noindex meta tag (which sometimes is ignored).

According to Google, "If you need to cancel your 90-day block from search results, you can visit the status page of the tool and click Reinclude next to a successful request. Requests take a few days to be process."

Try adding the site in Webmaster Tools with the other canonical form and check there as well for url removals.

Removal of the URL from Search Console doesn't influence indexing. It probably wasn't indexed in the first place.

You probably used Removal URL tool by accident.

    - Make sure you actually allow indexing (no noindex tag)
    - Then you must make googlebot to visit your website.
