PPC to survey monkey

Hi there,
hope you are well.. I have hit a brick wall with my current project
I set up a PPC campaign and got a good quality score.
When i created the survey using survey monkey they gave me a code to put on a webpage.
I then embedded the code on a webpage, then when I viewed the page I thought the survey would be viewed.. INSTEAD it just had a link there saying click here to take survey.. So I put a header, pic and some text inviting them to take the survey.
When I launched the campaign my quality score dropped to 1 on everything.. Because of a poor landing page..

How can I drive traffic using google adwords that when clicked takes the user direct to my survey monkey survey site and be able to maintain a good quality score?
