What Does This Mean? {Adwords}

With some keywords using google, you'll notice sometimes, with some of the adwords ads, that they'ell either have the state or city you live in right under the ad like so...

Does it mean the person/company advertising it, lives in "this case Maine"
or does it mean they are geo targeting maine with their keywords?

So anytime someone looks up "whatever" their will be regular ads and then a couple of with the city & state under them, is it cheaper to target
each keyword like that instead of going broad with it?

i.e if I were selling...um...black calculators, would it be cheaper/lower cpc
if I were to bid on "black calculators maine" vs' just "black calculators"?
and have ads targeting every city in the country or countries?
