Adsense Application Checklist

I think i have the site ready to apply with

1. its 100% unique
2. its a year old my domain that I own
3. It has privacy policy on it
4. The whois is my name address
5. The contact on the privacy policy is info@thatdomain.com

Am I missing anything
Do i need a link for contact us? or about us?
