It's simple just use the main keywords on the title, alt tag and on the description of the image, you don't have to do something more...

Image optimization is very effective process of improving visits to your site. It contains many factors like image name, alt text, redirect link, caption and image size. image name,alt text and captions should contain your targeted keywords.
