What Does Correct On-Page SEO Look Like?

As it stands, I make sure that the KW is at the very beginning of the title, in the first sentence, 2-3 times throughout the body and once in the last paragraph. However, I'm a bit concerned that I only write a 50 word introduction, including the KW in the first sentence, and then use the same keyword in H2. Can I get penalised for this?

Is it better to use the KW in the first sentence, H2, H3, once in the last parapraph, and sprinkle LSI throughout the body?
Is it good practice to bold the KW the first time it appears. Or is the SEO benefit from this minimal?

    It's not clear how long your article is ... and keyword density can vary depending on where the article is.

    and rather than use <b> bold</b> use <strong>strong</strong>

    I think I understand the basics, but I'm concerned about repeating the KW in H2, after already mentioning the KW a mere 50 words earlier in the introduction. Anyone?
